DVB-T is for terrestial reception, which is targeting digital TV and digital set top box.
DVB-H is for handheld, mobile reception. It basically is based on DVB-T frame work, adding 4096 sub-carieer mode at physical layer (better network planning), and MPE-FEC (mobile resilient), and time slicing (save power) at link layer (dig into the MPEG transport packets). DVB-H can be considered as an add-on for DVB-T.
